Failure mechanism? TCM programming error. The module could misinterpret electrical noise from the gear selector as a downshift command. So you’re cruising in “D,” steady throttle, maybe 60 mph — and the thing drops a gear on its own.
Sudden decel. Engine braking spikes. Driver confusion. That’s why it became a federal recall.
The fix was a software reflash. No hardware redesign. No clutch pack swap. No valve body teardown. Just updated calibration.
I’ve never seen one grenade internally because of this recall. Once reflashed, they behave normally.
Before buying one of these automatics, run the VIN. NHTSA lookup or Mazda’s site. If 3019A isn’t closed, you fix that before anything else. No excuses.
The Mechanical One: Early 2016 Manual Gear Issues

Main Failure Patterns
Early Manual 2nd / 3rd Gear Damage
This was real. I’ve seen dozens.
Driver complains about notchiness. Then grinding. Then it won’t go into gear at all. Sometimes you drain the fluid and it looks like metallic paint. Shavings everywhere.
